
Southlake Health is transforming into a Distributed Health Network, which includes an Advanced Care Centre in Georgina.
An Advanced Care Centre is a community-based facility providing outpatient healthcare services, which help to reduce reliance on local hospitals. This will allow Southlake to meet the needs of a growing and aging population, improve experiences for patients and staff, and bring care even closer to home.
Southlake Health has issued a Request for Expressions of Interest (RFEI) to identify potential partners to develop an Advanced Care Centre in Georgina. As a cornerstone of the Southlake’s Distributed Health Network, Advanced Care Centres will expand access to outpatient and ambulatory services, helping reduce the need for patients to travel for care while easing pressure on Southlake’s Davis Drive site.
In 2024, the Town of Georgina, Southlake Health, and the Northern York South Simcoe Ontario Health Team signed a memorandum of understanding to establish a formal relationship between the parties around shared goals of improving services for residents in Georgina. This work identified an opportunity to co-locate services — such as interprofessional primary care, diagnostics, outpatient clinics, and other community-based care — to better meet community needs.
The centre will support one of Ontario’s fastest-growing communities by improving access to timely, high-quality care and helping meet the evolving health care needs of residents for years to come. Through hospital partnerships and expanded community-based services, Southlake will enhance the patient experience, improve access, and build capacity across the region.
As planning and site exploration continue, Southlake Health will work closely with provincial and municipal partners, clinicians, staff, patients, and community members to help shape a model of care that reflects the needs of the communities it serves.
