Transforming Southlake Health
into a Distributed Health Network

Southlake Health's Distributed Health Network

Since opening its doors over a century ago, Southlake Health has delivered leading edge care, close to home to patients and families in northern York Region and southern Simcoe County. Building on that foundation of excellence, Southlake is on a journey to redefine how care is delivered.

Over the next decade, Southlake will build a Distributed Health Network (DHN) — a more sustainable model that organizes services across multiple sites and communities rather than relying on a single hospital. A new hospital in East Gwillimbury and redeveloped Davis Drive campus will anchor care across northern York Region and southern Simcoe County. They will be supported by community-based services at Advanced Care Centres across the region and enabled by digital health and system partnerships. Together, all of this will allow Southlake to meet the needs of a growing and aging population, improve experiences for patients and staff, and bring care even closer to home.

Guided by Population Health Management

Planning for the future of care in northern York Region and southern Simcoe County requires a clear understanding of the health and wellness needs of residents, how those needs are changing, and where future pressures are emerging across the local system. Southlake Health uses population health management to support evidence-base planning, coordination, and decision-making.

Population health management includes: 

​

• Understanding the current and projected health and wellness needs of Georgina residents 

• Examining how residents access and use health and community services 

• Identifying system pressures, service gaps, and opportunities for improvement

With provincial endorsement and funding secured, Southlake is now moving from planning to implementation of its Distributed Health Network. This milestone marks the beginning of the next phase of detailed planning for both the new hospital and the redevelopment of the Davis Drive campus.

Planning will focus not only on buildings, but on how care is delivered across the system. As part of the Distributed Health Network, Southlake will: 

​

  • Define services across two hospital sites 

  • Expand care through Advanced Care Centres, which will provide high quality hospital-level care and community-based programs 

  • Strengthen partnerships across the region to improve coordination and access 

 

The goal is to ensure patients receive the right care, in the right place, at the right time. 

Ongoing engagement will ensure the plan reflects local community needs, clinical expertise and innovation, and opportunities to improve patient and caregiver experiences.
